Jordan Health Services is currently seeking an outgoing, driven RN for a new Community Liaison role for the Livingston, TX area.Position Overview:
The Community Liaison is responsible for the recruitment of new patients, developing processes for patient referrals, and discharges. The Clinical Liaison also fills an important marketing and sales role for the organization as well as the RN clinical support in a designated area.Job Duties:Educate Physicians about services available through home health.
Maintain contact with referral sources.
Enhance awareness of home health services by interfacing with discharge planners, hospitals, and physician staff.
Be available to assist the community with wellness and prevention.
Participate in community-sponsored events and expositions.
Stay current with regulation changes in the home health industry.
Prepare education materials.
Know the company and the product lines.
Make a minimum of 50 calls (physician offices) every 2 weeks to develop relationships with physician offices, discharge planners, and other potential referral sources.
Reviews patient records, as necessary, for accuracy and quality.
Conducts OASIS and subsequent visits as scheduled.
Plans, delivers and monitors patient care by assessing personnel requirements to meet census.
Provides nursing care to patients, as needed.
Do other duties/projects as assigned. Position Requirements: Registered Nurse
Home Health Experience
Knowledge of Medicare/Medicaid Benefits, Rules & Regulations
Sales and/or Marketing Experience in a Home Health Setting strongly preferred
Ability to work independently and communicate effectively to community membersBenefits: $60,000 Starting Annual Salary
