Expanded Job Description
As the Licensed Practice Nurse, you will be responsible for:
Monitoring of patient statistics through technology and telephonic case management
Reaching out to patients to monitor their medications, disease management and diet
Intervening proactively, ahead of a costly acute episode
Using limited clinical resources more efficiently
Maximizing care coordination across the clinical continuum
Detecting biometric trends and patterns of non-adherence
Providing targeted and effective education
Required Skills
LPN
Home care experience
Patient education, diabetes, CHF, and cardiac experience
Strong clinical knowledge
Technically knowledgeable
