Sunlife Home Health is growing again! We are a Medicare certified and JCAHO accredited organization is seeking Nurses for positions in Home Health to join our fast growing and exciting organization. We are seeking RN's with experience in home health or hospital med/surge that have a proven track record of compliance and regulatory performance. Computer skill's is a big plus and a desire to become part of a family team specializing in a wide range of treatments, emotional support, and education to patients who are recovering from illnesses and injury.
• Admit patients to services and complete clinical documentation.
• Assess individual status and care / instruction needed by the patient.
• Confer with physician to develop the initial plan of treatment based on physician's orders and initial patient assessment.
• Provide hands-on care, case management and evaluation of the care plan, and education of the patient utilizing Allscripts point of care Software. Revise plan in consultation with physician based on ongoing assessments.
