REGISTERED NURSE POSITION DESCRIPTION
Service Area: Wayne, Oakland, and Macomb County
Position Title: RN
The Registered Nurse provides nursing services to Maternal Infant Health Program (MIHP) clients in the home and accepts accountability for his/her nursing practice. The RN assesses the MIHP client's health care needs and coordinates all services required for total case management.
Qualifications/Credentials:
1. All nurses must possess current Michigan licensure as a registered nurse.
A. Possession of a Master's of Science in Nursing (MSN) OR
B. Possession of a Bachelor's of Science in Nursing (BSN), and at least one year of experience providing community health, pediatric and/or maternal/infant nursing services; OR
C. Possession of a nursing diploma or Associate Degree in Nursing (ADN), and at least two years of experience providing community health, pediatric and/or maternal/infant nursing services.
2. Licensed driver with insured automobile which is in good working condition.
3. Possesses current CPR certification.
4. Satisfactory references from prior or current employers and/or professional peers.
5. Ability to pass Background Check
Responsibilities:
1. Assessment of client, family, and home environment (Requires Reliable Transportation).
2. Identifies problems, set goals, and establishes MIHP Plan of Care in consultation with client/family physician.
3. Refers to other disciplines and resources, as indicated.
4. Provides MIHP nursing services in accordance with physician's orders and agency & MIHP policies procedures.
5. Evaluated effectiveness and outcomes of MIHP Plan of Care on an ongoing basis.
6. Obtains and implements change in MIHP Plan of Care as ordered by the physician.
7. Instructs and supervise MIHP clients and their caregivers regarding nursing care.
8. Records and reports to the MIHP coordination & physician the client's response to treatment and/or changes in the client's condition. Reviews Plan of Care and Updates as needed.
9. Prepares assessment and progress notes, documenting client's progress and response to nursing intervention and teaching while planning for client's discharge form service.
10. Maintains communication with other disciplines to coordinate treatment goals and to support objectives outlined in Plan of Care. Informs physician & MIHP coordinator and other personnel of changed in client's condition and needs, as wanted.
11. Participates in In-Service, Continuing education programs, staff development activities, and Professional meeting.
12. Complies with the policies and procedures of Labor of Luv, LLC - MIHP.
