Overview
Nurse practitioners and "medical home coordinators" (specialty trained medical assistants) work together to deliver care to patients in their homes. While you are practicing medicine the Medical Home Coordinators help schedule home visits, assist the Nurse Practitioner in taking vitals, medical histories, performing diagnostic services and assessing patients' homes for safety and security. You will be assessing and managing chronic medical conditions: This includes care for all stages of life; acute care, chronic care, preventative services and end of life care and coordinating all products and services to achieve the highest possible outcomes at the lowest possible cost.

Typical Schedule is a 40 hour work week. Monday-Friday 8am-5pm, No call, No weekends.
Qualifications
Must be Certified as a Nurse Practitioner
Must have an active License in the State of Texas
Experience in Primary Care or Geriatric Medicine a plus
Recent NP graduates with experience in Critical Care or Emergency Medicine excel in our modelCompensation and Benefits
Competitive Salary based upon experience = $80,000 to $100,000
