JOB DESCRIPTION FOR RN'S - (SEE BELOW FOR JOB DESCRIPTION FOR LVN'S)
QUALIFICATIONS
• Graduate of a state approved school of practical (vocational) nursing and currently licensed in the state (s) in which practicing.
• Two (2) year experience in nursing, preferred.
• Ability to exercise initiative and independent judgment.
RN RESPONSIBILITIES
• Performs the initial evaluation visit and regularly reevaluates the patient's nursing needs.
• Initiates the plan of care and necessary revisions.
• Performs services in accordance with the plan of care.
• Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
• Coordinates services.
• Counsels the patient and family/significant others in meeting nursing and related needs.
• Participates in and presents in-service programs.
• Processes orders and notifies physician of patient needs and changes in condition.
• Completes certification/recertification orders and discharge summaries.
• Refers to Physical Therapists, Speech Pathologist, Occupational Therapist and Medical Social Worker those patients requiring their specialized skills.
• Supervises and teaches other nursing personnel.
• Conducts patient care conferences on patient assigned to his/her care.
• Is available for on-call duty nights, weekends and holidays, as assigned.
LVN RESPONSIBILITIES
• LVNs provide various health care interventions at home and provide health education the patients, families and caregivers living with the patient regarding the patient's current status and health needs.
• Home health LVNs monitors the vital sign of their patient such as BP, pulse rate, oxygen saturation, temperature and respiration rate. They also provide changing of wound dressings, administer injections, and provide alcohol rubs and back massages. In some areas, a home health LVN is permitted to hook intravenous fluids and any other medications.
All applicants must:
Have current license in the state of Texas
Ability to follow directions and provide excellent patient care
Ability to read, write, speak clearly and understand English
Meet all health requirements, as required by law
Relate positively, effectively, and appropriately with patients, residents, families, community members, volunteers and other facility staff.
Requirements:
• Must have at least 1 year of experience
• Current TX License
• Current CPR certification.
• Current TB Results or chest X-Ray
• Must be able to work irregular shifts.
• Must have a professional appearance
• No visible Tattoo's on body or facial piercing.
• Must have a clean criminal background
