CASE MANAGER -- Registered Nurse

AccentCare Home Health of Rogue Valleylocated in beautiful Medford, OR is in an exciting growth mode and has an immediate need for RN Case Managers. This fulltime, benefited position will have direct report to the Home Health Administrator/DPCS.

POSITION SUMMARY: Responsible for managing the care plans of the patients from admission through discharge and for maintaining the delivery of quality patient care. The position combines the activities of direct and indirect patient care and directs staff nurses, LVNs, CHHAs in the delivery of the individual patients' plan of care as well as identifying interdisciplinary needs and coordination of the allied health clinicians (OT, PT, SLP).

POSITION QUALIFICATIONS: This position requires that the individual possess a current Oregon license as a Registered Nurse, minimum of one year work experience as an RN (home health experience highly preferred with Oasis-C), reliable auto with proof of insurance and registration, valid Oregon driver license, and sufficient verbal and communication skills to perform required tasks. Current CPR Certification required.
