We continue to grow and we have a part-time opportunity (30 hours) in our Northern Worcester region for a Registered Nurse.
The RN will be part of a professional team that will provide support which includes assessment, planning, and care of our clients and their family members in the comfort of their home. The position will serve clients in our Northern Worcester region (Leominster, Fitchburg, Gardner, etc.). The RN will monitor the daily health and well-being of our clients via information provided by the caregiver using our web-based medical record system. The RN will also coordinate the implementation of physician's orders with the participant, caregiver and other team members. Position also includes training, support and evaluation of client caregivers.
Qualifications
Â• Minimum of two years' experience as a Registered Nurse in case management, care planning and assessment in eldercare/disability in a health care, long-term care, social service or community setting.
Â• Current MA License.
Â• Individuals who are fluent in Hmong, Vietnamese, or Spanish are strongly encouraged to apply.
Skills/Knowledge Required
Â• Knowledge of case management systems, community service and delivery systems for aging population required.
Â• Strong clinical and communication skills are essential with competent use of technology applications.
Â• Regular car travel within assigned regional geography is required.
