DUTIES: The incumbent serves as the Home Based Primary Care (HBPC) Coordinator whose supervising physician is the HBPC Medical Director or Acting HBPC Medical Director. This position is within the Geriatrics and Extended Care Service at the Oklahoma City VA Medical Center.HBPC provides for Veterans requiring complex care in the home. He/She serves both clinical and administrative roles assigned to the HBPC; prepares reports and action items; working in collaboration with Geriatrics and Extended Care Service medical directors, administrators, and the Associate Chief of Staff; participating on Medical Center, VISN, and National committees as needed; supervising HBPC nursing staff and the program support assistant; resolving complaints while improving customer service, educating staff, writing policies procedures and proposals; assisting with recruiting, hiring, and training; screeningconsults; and preparing for surveys and carrya portion of the patient loadof a full-time provider. This process includes employment verification, personal history, education & license/registration/certification verification, and reference check. (Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.)NURSE PRACTICITIONER:Education: Masters degree from a program accredited by the NLNAC or CCNE.Registration: Licensed or otherwise recognized as a nurse practitioner in a State and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body.Experience: Experience which demonstrated leadership in delivering and improving holistic care through collaborative strategies with others. Applicant will be expected to provide three (3) letters of reference.PHYSICIAN ASSISTANT:a. Citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Handbook, Chapter 3, Section A, Paragraph 3g.)b. The candidate must meet one of the following:(1) A bachelors degree from a PA training program which is certified by the Accreditation Review Commission on Education of the Physician Assistant (ARC-PA); OR(2) Graduation from a PA training program of at least 12 months duration which is certified by the ARC-PA and a bachelors degree in a health care occupation or a health-related science; OR(3) Graduation from a PA training program of at least 12 months duration which is certified by the ARC-PA and a period of progressively responsible health care experience such as an independent duty medical corpsman, licensed practical nurse, registered nurse, medical technologist, or medical technician. The duration of approved academic training and health care experience must total at least 5 years.c. Certification.
