Program Resource Coordinator / Nurse Case Manager (Brooklyn Park & Maple Grove)

Homeward Bound serves children, adolescents, and adults with a variety of disabilities, but we have the unique capacity of meeting the needs of those with the most severe and profound disabilities.
We are seeking two, full time, PRC / NCM's for our four person CAC homes in Brooklyn Park and Maple Grove.
This is a leadership position which manages staff and is responsible for the overall Nurse Case Management, Program and Operations.
Must have a current RN license in the state of MN and valid MN driver's license with a good driving record. Supervisory exp. preferred.
