South Shore Elder Services, a dynamic Human Services Agency, has the following opportunities available for caring, well-organized individuals who enjoy working with elders.
REGISTERED NURSE
PART TIME - 15 - 20 hours per week
No nights, No weekends, No holidays
As part of our continuing growth, The Clinical Assessment and Eligibility Department is seeking two part time RNs. A challenging position for an experienced RN to conduct visits in nursing facilities to assess residents' needs, determine clinical eligibility for nursing facility services according to Mass Health Regulations, and pro-actively assist nursing facility residents in their safe return to the community. Provide education to nursing facility staff, residents, and their families regarding service options available in the community.
In addition, RN conducts home visits to frail elders to assess and develop service plans to meet their specific needs in conjunction with an interdisciplinary team. Perform screening activities to determine clinical eligibility for Mass Health reimbursed services. Participate in the development of the Adult Family Care Program.
QUALIFICATIONS: Bachelor's degreee in Nursing or Human Services and a minimum of threee years clinical experience, one of which must have been in community nursing; or an Associate's Degree with additional two years community nursing experience or long term care direct service. Valid Mass R.N. license required. Excellent oral/written/computer skills and commitment to team work essential. Must be able to work independently and be self motivated. Must have valid Mass Class 3 Motor Vehicle license and provide own transportation.
SALARY: Commensurate with experience
