Registered Nurse Case Manager (20 hours per week)
Interested in working with seniors and assiting them remain in the community Springwell is growing and is in need of a RN to assess, develop and care plans for elders for services for personal care as well as program eligibility. Complete initial and annual assessment to determine care plan for any client in need of personal care as well as clinical eligibility for program enrollment.
Qualifications
Current MA RN license, a graduate of an accredited Nursing School/Program or a BSN. Previous experience working with elders/ adults with disabilities in the community preferred.
Russian speaking preferred.
Excellent computer skills required.
Should be comfortable using a laptop or PDA in the field.
