Home Health Case Manager/QA Nurse (West Chicago Suburbs - Woodridge)

CASE MANAGER/QA NURSE
The Home Health Case Manager/QA nurse evaluates and analyzes the initial evaluations visit to a patient on admission and regularly evaluate the patient's visit notes, orders, Plan of Care and physician's orders, Nursing Care Plan, and coordinate the health team services to provide well-organized and accurate documentation. This person will coordinates all health team services to provide well-organized and accurate documentation. The Case Manager/QA nurse will also manage nurses and cases to ensure accurate and quality care is being delivered according to accepted practices.
RN'S & LPNs
We are also looking for experienced Home Health nurses to provide skilled nursing care for patients in the surrounding West & South Chicago suburb areas. We are specifically looking to have a Registered Nurse cover the Cicero, Bellwood, Maywood, Oakbrook and other West & South Suburb areas. Must be willing to travel in the Chicagoland area and able to adapt to a growing patient base.
Bilingual (Spanish) is a plus for the Cicero area and will also be seriously considered.
