Our federally qualified health center, the first in Connecticut to be recognized by NCQA as a Level 3 patient centered medical home, seeks a dynamic nurse leader to advance the impact of our primary care nurse and ancillary clinical staff in their roles in clinically excellent care for all of our 130,000 patients and in expert care coordination for a smaller populations of focus based on health status, risks, and needs. The ideal candidate will hold a master's degree in nursing, have several years of progressive experience in primary care or community health, have relevant management and clinical experience, and be passionately interested in ensuring the highest quality of care for vulnerable and special populations. She/he will be well versed in technology, including use of electronic health records, have experience training both nurses and ancillary support to the standards of high performance in primary care, including those of the Joint Commission, and have the verbal, written, and analytic skills to participate fully as a member of the management team of the organization.
