EMARC Life Choices Program – providing services to individuals with disabilities, has a part time opening in day program.
Licensed Practical Nurse, LPN, will assist the Health Services Coordinator in the provision of healthcare services that assist individuals in attaining and maintaining an optimum level of wellness across their lifespan.
ESSENTIAL DUTIES* AND RESPONSIBILITIES
1. Provide health care services that assist individuals in attaining and maintaining an optimum level of wellness across their lifespan in conjunction with the Health Services Coordinator, RN *
a. This includes:
b. Ensuring daily management of medical needs and communication with residences,
c. Administering first aid when necessary, making sound judgments about care needed with individual injuries and illnesses, and providing follow up as necessary.
d. Obtaining vital signs and making appropriate recommendations for follow up care as needed.
e. Ensure that quarterly vital signs are obtained on all Medicaid funded individuals.
f. Ensure nursing documentation is completed.
2. Provide Medication Administration Management as needed by*:
a. Dispensing medications during program hours, ensuring appropriate documentation. Obtaining and maintaining current Doctor’s Orders for all medication dispensed during program hours.
b. Observing for side effects of medications and reporting any possible side effects to the individual’s residence and DDS Services Coordinator.
c. Requesting medications from residences. Receive and record on medication count sheets. Process new medication orders and edit monthly medication orders. Dispose of any expired medications with another nurse.
d. Completing Narcotic Count daily with nurse or administrator.
3. Provide Personal Care for participants following clinical team and doctor recommendations/protocols including required nursing procedures i.e. catheterization.*
4. Provide backup support for LCNS or LCMN Health Service Coordinators as needed.*
5. Facilitate training for staff instilling a clear awareness of Health and Safety fostering the development of staff knowledge and skills, while demonstrating through action the kind of behavior which fosters an individual centered environment. Assist in the development of trainings for individuals receiving services based on developmental skill need areas and other regulatory requirements as it relates to health services. *
6. Participated and assist with intake process and orientation of new individuals to Life Choices health care services.
7. Maintain customer focus at all times in the delivery of services. Foster an environment which is committed to quality, responsiveness to participant and family needs, and professionalism in participant and family relationships.
8. Perform job duties in close cooperation with other departments fostering a work environment of mutual cooperation and support. In cooperation with Health Care Coordinator and clinical staff, make recommendations for policies, procedures and program needs in accordance with the mission of EMARC.
9. Ensure compliance with CARF, DDS, and Medicaid regulations, as well as EMARC’s policies and procedures. Provide follow up to all incident and accident injury reports related to individuals receiving services as necessary.
10. Assist in maintaining a clean and safe environment by reporting issues to Life Choices Director.
11. Other duties as assigned within the scope of Licensed Practical Nurse Position.
QUALIFICATIONS
Massachusetts LPN licensure. Valid driver’s license required.
1. Excellent verbal and writing skills.
2. Good ability to communicate effectively with people.
3. Good analytical ability and good organizational and management skills.
4. Flexibility, poise, tact and the ability to develop rapport with individuals of various backgrounds and positions.
5. Good knowledge of law and regulations pertinent to the Agency plus familiarity with generally accepted principals regarding accounting, business, personnel and management.
6. Acceptable CORI report
7. Valid Driver license, acceptable driver record and reliable transportation, Location: Reading, MA This is a part-time job.
