RN Case Manager

Job Description:
Registered Nurse to provide case management to the elderly and disabled in their homes. Responsibilities include but are not limited to client/patient assessment, plan of care development, field staff supervision, and coordination of services.
Qualifications:
1. Current RN license in this state, or permit to practice and a year experience.
2. Able to supervise (instruct, assign work, etc) LPN's, Nursing Assistant, and PCW's.
3. Valid driver's license and reliable transportation and proof of insurance.
4. Ability to work independently.
5. Excellent communication and listening skills.
6. Compassionate, patient and caring nature.
7. Able to work in a stressful environment.
