Intake RN Positions (Bronx, Manhattan, Queens)

We have immediate needs for Intake RN positions for positions in hospitals in Bronx, Manhattan and Queens.
The main function of position is to complete initial assessment/intake of patients when they are first admitted into the hospital, as well as to oversee and coordinate discharge planning services for the patients that are being referred into homecare.
Requirements:
- Current and valid NYS RN Licensure
- 2+ years of homecare experience
- Prior experience in discharge planning (making referrals into homecare) or case management (specifically, coordinating with an interdisciplinary team to plans needed medical and homecare services for patients)
- Excellent written and verbal communication skills
- Excellent follow-up skills
- Ability to work independently, as well as in a team setting
- Good documentation skills (experience with 485s required; experience with OASIS strongly preferred)
Compensation:
$45.00/hr. This is a temp. - perm. position.
