Walker Methodist Health Center is recruiting for a full-time RN Transitions Manager. We seek to maintain strong clinical partnerships with our community and primary care customers to ensure an easy and safe transition for our current and future residents. The Transitions Manager will ensure all needs and wants for the resident’s transitions are being met while working with the leadership team to pro-actively address any existing barriers. The Transitions Manager will collaborate with the Walker Methodist team and community partners to provide our customers with a significantly higher level of customer service. Extensive local travel.
Successful candidates must have:

• Commitment to assisting seniors in developing their fullest potential
• Thorough knowledge of the continuum of care and community resources
• Sensitivity to older adults’ needs and the ability to work with them and their families in the transition between settings
• Excellent customer service and communication skills
• Ability to prioritize and make independent decisions
• Current MN RN license and BLS-certified with five years of experience, preferably in case management
• Working knowledge of state rules, regulations and compliance requirements
