Registered Nurse Case Manager (Pittsfield, MA)

Energetic, focused, caring, well-organized R.N. to work with individuals across many aspects of the care continuum. Responsibilities include completing in-home assessment of service needs for frail individuals, providing health education, collaborating with area nursing facilities to plan the safe return of individuals to the community, providing ongoing health monitoring for a variety of individuals and determining clinical eligibility for MassHealth-funded services. Excellent written, communication, and computer skills required. Geriatric and/or community-based nursing experience preferred. No evenings, weekends, holidays or on-call. Flexible part time hours available.
