POSITION SUMMARY
The LPN/LVN supplements the nursing care needs of the patient as provided by the RN and as specified in the plan of care. Provides services in accordance with organizational policies and procedures and regulations that define scope of practice.

Essential Functions Statement(s)
Prepare clinical notes documenting outcomes of interventions.
Assist the RN and/or physician in performing specialized duties related to end-of-life care.
Assist the RN in carrying out the plan of care.
Prepare equipment and materials for treatment adherring to aseptic technique as required.
Assist the patient in learning appropriate self-care techniques.
Assess patient/family response to care.
Ensure communication of information to appropriate team members.
Complete annual on-going training/continuing education.
Assist with special projects and other responsibilities as assigned.
Education: Graduate of an accredited school of nursing that leads to LVN/LPN
Experience: 1-3 years of related health care experience
Computer Skills
To perform this job successfully, an individual should have knowledge of Internet software, Work Processing software, Healthcare software.
Certificates and Licenses
Current Licensed Practical Nurse or Licensed Vocational Nurse in state where work is performed with no restrictions.
