RN Case Manager (West Valley)

Sun Valley Hospice is looking for an RN Case Manager in the West Valley to join our growing company. This is a full-time position (Monday -- Friday, 8-5pm), plus occasional on-call hours.
RN Case Manager duties include:
- Direct patient care
- Documentation
- Coordination of interdisciplinary care team
- Supervision and assignment of Hospice Aides
- Attendance at IDT/IDG meetings in East Mesa
- Participation in education activities
Qualifications:
- RN licensed in the State of Arizona (hospice experience preferred.)
- Excellent patient assessment skills
- Demonstrated organizational and time management skills
- Be highly self-directed and independent
- Have excellent communication and clinical skills
- Able to use current technology, such as e-mail, Blackberry, and eventually transition to using point-of-service documentation.
