Part-time Hospice RN - Case Manager (Portland Metro Area)

Hospice Registered Nurse -- Case Manager -- Part-Time, 32 hrs/per week.
Housecall Providers is a unique non-profit medical practice dedicated to serving the needs of home-bound individuals providing in-home primary medical care and hospice services.
The schedule for this position will be 32 hours per week, days to be determined.
We are currently looking for a Hospice RN/Case manager who will be responsible for professional nursing services of hospice patients as indicated by the plan of care developed by the interdisciplinary group. Responsibilities include initial and on-going assessment of patients, developing and revising the plan of care, timely documentation and communication of assessments, planning and interventions and coordination of care among team members. Will make initial and on-going visits to assess patient and provide baseline and comprehensive data. Requires utilization of the nursing process to provide patient care that is individualized, goal directed and consistent with current standards of practice.
You will also have responsibility for developing and communicating the plan of care for the hospice aide including visit frequency while evaluating the effectiveness of the hospice aide plan and revisions as needed. Provides and documents the required supervision of the hospice aide.
Additionally, you will identify and communicate the needs of the patient and family to the interdisciplinary group, the hospice medical director and primary care provider as well as be a core member of the Interdisciplinary group. Will participate in continuing education and assist with orientation of other team members.
Will maintain infection control and aseptic practices while following Housecall Providers employee policies and procedures. Must maintain high standards of professional practice by adhering to the ethics of the profession and keeping abreast of the latest professional practices.
Must have the ability to relate to patients and families in an open, empathetic and supportive manner while maintaining personal and professional boundaries.
Required participation in quality assessment and performance improvement (QAPI) activities.
Other duties as assigned by the Hospice Clinical Supervisor.
Qualifications:
Education, training, and experience:
Graduate of an accredited school of nursing. Bachelor of Science in nursing desirable. Unencumbered Registered Nurse License in Oregon. Two years of hospice, palliative care, oncology and/or home health experience preferred. Valid driver's license and car insurance.
Knowledge, skills, and abilities:
Ability to communicate effectively with diverse staff and patients, their families and caregivers and the community at large. Ability to conceptualize, plan and implement widely diverse job duties that require attention and coordination. Computer literacy. Demonstrated problem-solving skills. Sound judgment and integrity are non-negotiable personal qualities. Ability to work within a team and independently with minimal direct supervision.
This individual must have the ability to communicate effectively with diverse staff, patients, their families and caregivers and the community at large. Must have the ability to conceptualize, plan and implement widely diverse job duties that require attention and coordination. Must have computer literacy with demonstrated problem-solving skills. Sound judgment and integrity are non-negotiable personal qualities. Ability to work within a team and independently with minimal direct supervision.
Mileage and some cell phone reimbursement included in compensation package.
