Full-time RNAC/MDS Coordinator (Rolling Hills Health Center)

Rolling Hills Health Center has a full time RNAC/MDS Coordinator position available. Applicants must have a current RN license and have a passion for excellent resident care.
Job Description: Employee is required to perform an assessment of each resident's needs in accordance with company policies and procedures, standard nursing practices, and governmental regulations so as to maintain quality care for all residents.
Accountability: The MDS+ Care Plan Coordinator reports to the Director of Nursing.
Equipment used: Various office equipment and various medical equipment.
Duties and Responsibilities: The employee must be able to perform the essential functions of the job with or without reasonable accommodations. Except as specifically noted, the following functions are considered essential to this position.
1. Conduct initial and periodic comprehensive, accurate and standardized assessment of each resident in the facility.
2. Develop and maintain schedule system to ensure all assessments and care plans are conducted in accordance with guidelines set forth by governmental agencies.
3. Complete RAPS and Triggers according to schedule for each resident.
4. Perform quarterly assessment review and perform revisions as appropriate per each resident condition.
5. Certify resident assessment has been completed.
6. Lead interdisciplinary team to develop a comprehensive care plan for each resident, which shall include measurable objectives and time-tables to meet resident's medical, nursing, mental and psychological well-being.
7. Review care plans as requested with appropriate parties.
8. Review care plan objectives as necessary to ensure appropriate care and service for each resident.
9. Assures personal and staff compliance with all State and Federal regulations including blood borne pathogens, infection control, hazardous materials, fire safety, and building, electrical, and plumbing codes.
10. Assures personal and staff compliance with residents' rights.
11. Required to perform quarterly fall risk assessment update.
12. Required to check weights on weekly and monthly schedule.
13. May be required to cover floor as charge or staff nurse and to take rotate on call status.
Other Training, Skills, and Experience Requirements: Employee must be able to perform in such a way to build good working habits and relationships among the staff that they manage as well as other departmental staff. Employee must demonstrate on an ongoing basis the ability to develop and maintain good working relationships with co-workers and other department.
Health, Dental and Vision insurance offered. Wages will be based off years of experience.
