RN Case Manager (Bloomington)

Professional Resource Network is currently hiring a Full Time RN Case Manager in our Bloomington office. This position will cover the south of the river territory (Apple Valley, Eagan, Burnsville, etc) and some travel to those locations will be required.
This position is Monday through Friday 8am-4:30pm, however some flexibility on the start and end times is an option. Approximately 80% of your time will be spent in the office location and 20% in the field visiting clients. Most of our Case Managers also utilize a work from home day 1 time a week.

DESCRIPTION:
Provides leadership in the provision of nursing care to the Professional Resource Network Home Health client. Admits clients and coordinates their home care course of treatment. Confers and collaborates with physicians in implementation of the home care treatment plan.
REQUIREMENTS:
1. Currently licensed as a Registered Nurse in Minnesota.
2. Holds a valid Driver's License.
3. U.S. citizenship or evidence of a valid alien work permit.
4. Minimum of three years of adult medical-surgical experience.
5. Minimum of one year home health care experience.
6. Excellent organizational and management skills.
7. Knowledge of generally accepted principles of accounting.
ACCOUNTABILITY:
Reports directly to Director of Nursing.
QUALIFICATIONS:
Knowledgeable of Medicare, regulatory and credentialing criteria and standards. Excellent written and verbal communication skills. Able to utilize a telephone for internal and external communications. CPR certified. Competent in care provision in area of professional licensure.
WORKING ENVIRONMENT:
Office environment. Potentially unsafe and unsanitary home environments. Potential travel in inclement weather.
PHYSICAL REQUIREMENTS:
Driving or riding in a motor vehicle, standing, sitting, walking, bending, reaching, and stretching. Lifting up to 50 pounds unassisted and 325 pounds utilizing an assistive device.
Providing after-hours, weekend and holiday on-call coverage.
RESPONSIBILITIES:
1.Assists with the coordination of client care staff and services.
2.Completes in-home admission and assessment visit within established time parameters.
3.Develops and implements an individualized client plan of care.
4.Submits documentation of all client care within established time parameters.
5.Completes home care certification (485) forms and recertification (485) forms within established time parameters.
6.Reports progress and changes in client condition to physicians and referral sources on an on-going basis.
7.Provides clients and caregivers with referral to appropriate community resources.
8.Develops and implements a comprehensive teaching plan for all clients within the caseload.
9.Initiates and updates drug profile and medication teaching plan for all clients within the caseload.
10.Begins discharge planning at time of admission to home care.
11.Provides care according to the policies, procedures, and protocols of the organization.
12.Evaluates effectiveness of interventions and progress toward client goals.
13.Preceptors new home care clinical staff and assists in training, competency testing and orientation.
14.Participates in quality improvement, utilization review, and multidisciplinary conferences.
15.Reviews clinical records for accuracy and completeness.
16.Attends staff meetings and in-services.
17.Serves as a resource and consultant to the home health field staff.
18.Supervises non-licensed staff and licensed practical nurses in the provision of client care.
19.Demonstrates a high degree of commitment to customer service and quality care provision.
20.Exhibits good written and verbal communication.
21.Evaluates supervised staff informally and formally with written performance appraisals upon completion of their probationary period and on an annual basis.
22.Completes other duties as assigned or requested. Location: Bloomington
