FAMILY NURSE PRACTITIONER
Come join our family at NARA NW and work in collaboration with a multi-disciplinary team to provide high quality patient-centered care. We provide services for all ages at a number of sites. The person in this position will provide comprehensive primary health care in the NARA Indian Health Clinic and at the Residential Treatment Facility (A&D Treatment). Will coordinate patient care with other providers and agencies and advocate for the client within health care systems. Performs initial and ongoing health risk assessments and counseling for high-risk lifestyle behaviors and/or familial diseases. Develops and documents plans of care and outcomes evaluations. Participates in on-call service coverage, as needed.
TO QUALIFY: Must have current, unencumbered licensure with State of Oregon as a Certified Nurse Practitioner with prescriptive authority. Evidence of experience sufficient for successful performance of duties noted above. Prefer experience with diverse communities and low-income families. Must have exceptional interpersonal, verbal, and written communication skills and the ability to engage and interact positively with consumers to promote strengths and improve health and well being and to work collaboratively as part of an integrated team to coordinate care.
