Case Manager RN Hospice Needed Now.
IMMEDIATE CAREER OPPORTUNITY!
We are a team of experienced hospice professionals looking for a Part Time and Full Time opportunity. Currently looking to fill for our Galesburg, IL location with in Beacon of Hope Hospice.
The Hospice RN is responsible for performing direct patient care, including evaluation of needs and conduction of case and resource management, under the direction of a licensed physician.
Reports information back to management through written or phone correspondence.
Provides consistently high quality care to patients in accordance with the plan of care and Agency policies.
Communicates regularly with other disciplines involved in care, and evaluates needs for other services, making recommendations when appropriate.
Submits accurate and timely documentation of care to appropriate personnel.
Effectively manages the patient caseload that has been mutually agreed upon.

Requirements:
Hospice or home health experience preferred.
2+ years of nursing experience.
Other requirements may vary by case or department needs.
Graduation from an approved educational program for Nursing.
Active RN license and current CPR certification required.
