Looking for Nurses and Geriatric Care Managers to Join Rapidly Expanding National Network in Chicago, Illinois
SeniorBridge is a national care management and home care provider.
We are looking for nurses and geriatric care managers to be a part of our Transitional Care Program working with clients to reducing hospitalizations and emergency room visits among people with chronic illness and disabilities. This new program will open significant opportunities for individuals to grow their business by joining the SeniorBridge Care Management Network.
