RN/LPN - 4 days and e/o wkend, days (8-4:30) (Smithfield - Asst'd. Living)

RN or LPN needed immediately for very busy Wellness office at Atria Senior Living, Lincoln Place - Smithfield RI.
Position is per diem, very likely to perm; days 8a.m. to 4:30p.m., (4 days) and e/o weekend, some holidays, same shift totalling 5 days/week. Experience w Assisted Living, elderly and or dementia care are preferred. We have residents who are assisted living only as well as residents on a secure dementia/Alzheimer's care unit. Training will be provided for dementia care.
Strong assessment and organizational skills mandatory. Oversee med officeof 1 Med Tech and 2 CNA's. Take incoming calls, MD orders, transcribe and chart. Monitor communication log and complete tasks as needed. Communicate w/ MD's and family when medical change of status occurs. Complete medication change orders, refills and coordinate with pharmacy. Coordinate outside skilled nursing, PT services and medical appointments for residents. Assess resident medical complaints or changes reported by staff, no medical treatment provided on site.
Coordinate emergency send outs with family/local 911/ambulance as needed. Assist Nursing Director w tracking, projects, scheduling, TB testing of staff/residents, fall risk assessments, nursing notes, follow ups, ability of residents to self medicate, and other state and internal protocols. Must be hard working, have an unrestricted RI license, and be looking to grow with a nationally recognized senior living organization of over 120 communities across the U.S.
Hourly rate is determined by licensure and skill/experience. Salary requirements MUST be included with your resume.
