RN Case Manager $33-$36 hourly (Macon )

Staff Easy is looking for an RN:
The qualified candidate needs to possess the knowledge, skill, and ability to assess patients, evaluate, update and follow through with the patient's progress and care goals. Duties include assessing and monitoring patients, determining eligibility for treatments, determine whether the plan of care being provided meets the patient's needs, and arranging for additional services as needed in order to help patients achieve desired outcomes. Other duties include analyzing data, and coordinates patient care with all members of the patient's health care team, providing each patient with individual planning and planning in order to ensure optimum outcome while also seeking to ensure care and services provided are appropriate to patient needs and cost effective. Salary is $31.00-$35.00 hourly.
Requirements for the ideal candidate are;
Bachelor's Degree in Nursing
RN Case Management Experience helpful
RN license with no restrictions
CPR and Immunization Requirements
Must read, write, and speak English well enough to effectively communicate with patients and staff
Adequate computer skills
Current Background Check Helpful
Valid auto insurance
