RN Staff Case Manager

Providence is calling a Case Manager RN for the Case Management Department at Providence Alaska Medical Center in Anchorage, AK.
Under supervision this position facilitates coverage of several different positions during the course of each workday including Admission Case Manager and RN Case Manager.
In this position, you will:
Plan and provide discharge planning and other services to address patient/family needs identified in assessment.
Match patients with appropriate internal and external resources to meet their ongoing needs, and facilitates application(s) for programs and referrals for special needs.
Perform integrated concurrent review to identify quality and utilization issues.
Apply admission, continued stay/transfer, and discharge criteria to each patient's case.
Interface with external review companies and consultants, using advocacy and negotiating skills that address medical necessity and appropriateness of inpatient/outpatient levels of care.
Assist in management of resources with informational and monitoring tool(s) that support access, confidentiality, assessment, analysis of care and throughput, intervention, efficiencies, and quality of care.
Review physician orders for observation and admit to Inpatient status.
Initiate discussion with the physician when patients do not meet appropriate criteria for ordered admission status.
Qualifications for this position include:
Graduation from an accredited school of nursing.
Licensed as Registered Nurse in the State of Alaska.
Two years of utilization review or case management nursing experience required.
Successful candidates will possess thorough knowledge of provider requirements for admissions, transfers and discharges.
Must have thorough knowledge of InterQual criteria, Medicare standards and COBRA regulations.
Must have ability to communicate (written and verbal) well and interact with multiple health care professionals, including medical and allied staff.
Must be able to work with ill, disabled, elderly, and emotionally distressed patients/families.
