Home Care RN Case Manager
Per Diem or Part Time Home Care RN case manager. Monday through Friday 8:30-5 pm.

Our nurses have the autonomy to provide hands on care, case management, evaluation of the plan of care, and education to our patients and families on a one to one basis. We are a Medicare/Medicaid Certified, Joint Commission Accredited (JCAHO) Home Nursing Care Provider.
We offer competitive pay rates, flexible, family friendly schedules, and no mandatory overtime.
1-2 years of Home care experience required
RN licensure in RI is required
