JOB SUMMARY:Vacancy Identification Number (VIN): 596544Â(Include #Âon all documents)Duties/Responsibilities:There is an exceptional opportunity for a Nurse Practitioner with expertise in Hospice and Palliative Care to join our team of professionals caring for veterans at the end of life. VA Maine opened a beautiful 12 bed hospice unit in June of 2011 and is now seeking a mid-level provider to compliment our team in providing end of life care to veterans in the inpatient hospice unit. In addition the provider will follow veterans on community hospice and serve as a member of the palliative care consult team intervening with veterans and their families in all settings within the VA Maine Health Care System.Disabled persons and/or veterans are encouraged to apply.The vacancy requires well organized Nurse Practitioner with the ability to work autonomously and collaborate with in-clinic, in-Medical Center, community and the New England VHA Network services to best-serve theÂpatient’s needs.Competitive salary, generous leave package, health & life insurance benefits, and retirement benefits. Facilitates and promotes the delivery of high quality care to patients at the end of life and their significant others.2. Provides leadership in the implementation of current palliative care standards for nursing and allied healthcare staff.3. Participates in the development of a comprehensive, individualized plan of care based on assessment of the patients palliative care needs.4. Recommends change in practices based on analysis and evaluation of research findings, current regulatory standards and applicable laws and regulations in coordination with VHA National Hospice and Palliative Care Programs and directives.5. ÂSafely and effectively uses equipment required for patient care using hospital policies and procedures, including appropriate cleaning and disinfection of reusable medical equipment.ÂETHICS: Provides leadership in addressing ethical issues in end of life care that impacts veterans, families, caregivers, and clinicians.a. Assures competent and consistent delivery of palliative care to eligible veterans.b. Serves as a resource and advocate for veterans in need of palliative care services.ÂRESOURCE UTILIZATION: Manages program resources (financial, human, material and informational) to facilitate safe, effective and efficient care.Âa. Promotes and implements cost-effective use of resources.b. Promotes and utilizes information systems in place to facilitate planning and management of palliative care program.c. Explores alternative solutions to problems and selects the most appropriate, efficient and effective approach.Âd. Demonstrates knowledge of fire/emergency population.3. Describes review of patient care plan with less experienced staff to address ethical issues for client and/or staff, e.g. a patient with advanced illness who wants to elect hospice care confidentiality, self-determination, and alternative treatments.5. Demonstrates care in coordination to use resources judiciously and achieve optimal outcomes for patients/families.6. Serves as a consultant/resource to other advance practice nurses related to application of evidence-based information to the design, implementation and evaluation of a treatment plan7. Provides an example of the design, implementation and evaluation of an education program for a group of patients/families or specific target population including outcomes achieved.8. Synthesizes data and information to prescribe necessary system and community support measures including environmental modifications.9. Develops solutions for patient problems that address effective resource utilization and maintenance of quality care, such as description of program to reduce visits through prompt telephone hot-line access with demonstrated improved outcomes.ÂEDUCATION: Assists palliative care staff to maintain and improve palliative care expertise.Âa.  Maintains professional knowledge and skills based on currently publications or communicated information.b. Identifies current trends in clinical practice and palliative care modalities.c.  Assesses needs of diverse groups of staff, patients and families to maximize learning and compliance with program objectives.d. Implements an educational plan to meet
