Brightstar (Hartford, CT) is currently seeking a RN to be the Nurse Manager for our Non-Medical Home care Agency. The Registered Nurse RN is responsible for providing home health clinical services along with quality, adequacy of services provided and may, where required, supervise staff. The staff registered nurse RN is responsible for ensuring that care is delivered appropriately and the standards of quality are adhered to. The staff registered nurse RN is responsible for working with other medical Agencies and Personnel to ensure coordinated services and that all of the client's needs are identified through the assessment process and addressed through the implementation of the Plan of Care and reevaluation and supervisory process. This is a Salaried position (Flexible 30 Hours per Week) with Benefits (Medical, Vacation + Bonus Eligible). Must Manage On-Call services only (1) weekend a month.

•Initiates the plan of care and makes necessary revisions.
•Prepares clinical and progress notes and summaries of care.
•Informs the physician and other personnel of changes in the patient's condition and needs.
•Counsels the client and family in other Medical Agency meeting care plan and related needs.
•Assists in the development of organization goals. Administers agency policies and procedures.
•Assesses the competency of clinical staff to promote quality, continuity and safety of a client's care.
•Supervises, educates and coaches other nursing personnel, as directed..
•Provide information to be used for statistics, reports and records for purposes of evaluation and reporting of organization activities.
•Assures proper maintenance of clinical records in compliance with local, state and federal laws.
•Assures compliance with all local, state and federal laws.
•Identifies and participates in agency performance improvement activities.
•Stays informed about changes in the field of nursing and home health care. Shares information with appropriate organization personnel.
•Promotes home health referrals in the health care community.
Qualifications:
•Registered Nurse with current unencumbered licensure to practice professional nursing in the State of Connecticut is a must (MA & CT licenses preferred) in which the care is being provided .Candidate must have a Bachelor's of Science Degree in Nursing with a minimum of two (2) years of documented experience in Homecare/Hospice or experience. .
•Comply with accepted professional standards of practice.
•Excellent interpersonal skills.
•Analytic/Critical thinker
•Excellent customer service skills and ability to deal tactfully with clients, staff and community.
•Treat clients, staff and the public with courtesy, respect and presents a positive public image.
•Case management skills are a plus.
•Ability to travel on a daily basis, manage on-call services only one (1) weekend a month
