RN Case Manager Needed for Home Health Care Assignment (Kenosha, WI)

Firstmed is currently recruiting for an RN to work as a Case Manager for a 3 month long Home Health contract.
As a Case Manager, you will coordinate all aspects of the patient's home care, while providing professional care to the patient. The RN case manager conducts ongoing assessments to identify problems and modify a nursing care plan geared specifically for the patient. You will be responsible for completing all necessary documentation of visit assessments to the client's standards.
Hours for this position are Monday-Friday, from 8 AM-4:30 PM, for 3 months, with the potential for a permanent position. Pay for this position is based upon experience.
Requirements:
Â• RN licensure in the State of WI
Â• BLS Certification, ACLS Preferred
Â• 1-2 years of recent home health experience
Â• Excellent Oral and Written Communication skills
Â• Ability to give and take direction and able to deliver exceptional care
Â•Med/Surg and ER Experience a plus
