JOB TITLE: Resident Care Coordinator
DEPARTMENT NAME: Nursing

OVERALL RESPONSIBILITIES:
Serves as the communities registered nurse. Responsible for resident assessments and service plans. Responsible for medication overall management. Responsible for staff training and development. Is responsible to ensure compliance with State, and other legal entities for the services provided to residents and their families.
WORKING RELATIONSHIPS
Reports to: Executive Director
Supervises: Nurses and Medication Technicians if applicable
Interfaces with: Residents, families, visitors, and associates
PRIMARY JOB DUTIES
Conducts resident assessments in accordance to State rules and community policies.
Completes and maintains current resident service plans. Service plans must be unique to the resident and based on the most recent assessment. Communicates to appropriate staff the content and requirements of the resident service plans.
Develops and maintains community drug management programs. Trains and monitors nurses and unlicensed staff in the administration and management of medications.
Develops staff training programs and trains staff. Arranges for training from outside vendors. Ensure compliance of all community staff with state training requirements.
Assists with managing census to maintain stabilized occupancy and proper levels of care.
Develops, implements, and monitors the community's Employee Health Program. Ensures compliance with State regulations.
Ensure compliance with all rules, regulations, laws and statutes pertaining to the licensing of an assisted living community.
Maintains and protects the confidentiality of resident information at all times.
Performs other reasonable tasks as assigned by supervisor.
JOB QUALIFICATIONS
Must be a registered nurse in the State of Utah.
Ability to effectively communicate with residents, families, supervisor, employees.
Willingness to work with the elderly. Location: South Jordan, UT
