HCI Care Services, formerly Hospice of Central Iowa, has full-time R.N. opportunities in our Boone/Perry office. This position will focus on providing quality home-based or facility-based patient care by partnering with interdisciplinary team for end-of-life care plan services. The responsibilities will include determining and adjusting care plans based on patient assessments and condition changes, partnering with physicians & pharmacists for pain management, assigning care plans to interdisciplinary group, following progress of care plan assignments and preparing for re-certification requirements. The service area includes the counties of Boone, Dallas, Greene, Guthrie, Hamilton, Story and Webster. Candidates must have current R.N. license, 2-yrs clinical exp in med/surg or oncology preferred, electronic charting exp., and have independent / autonomous work ability.
