Angels Care Home Health is currently looking for a RN Case Manager for our home health branch in Tahlequah, OK.
Primary Responsibilities:
*Administer skilled nursing care for clients in their place of residence.
*Case manage and provide clinical directions to the interdisciplinary team, patient/ family and referring agency; and assume the responsibility for coordination of care.
*Assure continuity of quality patient care is delivered with appropriate documentation.
*Demonstrate commitment, professional growth and competency.
Requirements:
*Must be a graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
*Must hold a current Oklahoma State license as a Registered Nurse.
*Must hold a current Oklahoma State Driver License.
*A minimum of 6 months experience as a Registered Nurse in a clinical care setting, home health preferred.
*Good interpersonal skills, proof of current CPR and Hepatitis Profile, reliable transportation with current, valid auto liability insurance.
