Wellness Nurse
(RN)
This position is part time and approximately 18 to 22 hours per week.
The Visiting Nurse Association is excited to announce the opening of a per diem position for a SASH nurse on our Senior Housing Wellness Team! Provide a variety of clinical services with the goal of enhancing, promoting and maintaining the health of residents living at senior housing facilities.
The functions of this position include:
• Facilitating safe and effective transitions for patients between off-site care facilities and home.
• Encouraging and promoting the importance of self management of chronic health conditions.
• Conducting health and wellness assessments, health education, foot care and medication management.
Qualifications:
• Current RN license with at least two years of experience in a clinical setting.
• A commitment to an aging in place philosophy.
• Must be able to work independently and possess strong organizational and time management skills and excellent verbal and written communication skills.
