Currently looking for part time Nurse Practitioners to complete health risk assessments on plan members in the home setting and skilled nursing homes.
Responsibilities include:

• Field based position completing health assessments for individuals in their homes and nursing home facility
• Ensure the completed assessments are legible and accurate
• Identify patients that may meet disease management programs
• Maintain open lines of communication with an emphasis on supportive relationships with patients and health plan staff
• Strong professional level of knowledge and comprehensive clinical assessment skills in the elderly population and chronic disease management
• Prior experience as a nurse practitioner in a long term care setting
• Strong decision making skills
• Self starter
  Requirements:
  Must have 3-5 years of experience
  Must be a FNP, ANP or GNP
  Must be a licensed NP
  Must have a Masters Degree
  Must be a APRN-C
  Must be certified by ANCC or AANP
