Full-time Hospice Nurse (Missoula)

Hospice of Missoula is actively recruiting a Full Time RN Case Manager.
Applicants must live in the city of Missoula, have reliable transportation, and be willing to take call.

RESPONSIBILITIES AND FUNCTIONS
A. Professional and clinical services:
1. Perform new patient intake assessments, including physical, psychological, social, and spiritual parameters. Patients are assigned by the PCC.
2. Develops or assists with the development of a comprehensive plan of care for patients based on the assessment.
3. Makes appropriate referrals to other community agencies as determined by patient/family needs and resources.
4. Coordinates care with the Hospice team, i.e., Hospice staff, primary physician, volunteers, clergy and patient/family.
5. Supervises and/or provides direct patient care within the guidelines of nursing practice in the State of Montana and as defined by HOSPICE OF MISSOULA.
6. Provides bereavement follow up and/or referral within the guidelines of
Hospice's bereavement policy.
7. Shares in 24 hour call coverage as assigned by the Director. Call coverage is for all active Hospice patient/families and new referrals received.
8. Assists in the selection of Hospice volunteers for specific patient/families. Introduces and supervises the volunteers, when necessary.
9. Attends volunteer support and educational meetings as schedule allows.
10. Attends a minimum of six staff in-services per year.
B. Educational and outreach:
1. The staff nurse will assist in public education presentations when appropriate.
2. The staff nurse will assist in volunteer training presentations as needed.
QUALIFICATIONS
1. Graduate of an accredited registered nursing program.
2. Demonstrates knowledge of an ability to apply sound nursing principles to Hospice care.
3. Demonstrates ability to supervise volunteers.
4. Maintains current RN licensure in the State of Montana.
5. Skills required:
a. Interpersonal skills
b. Competent home nursing care skills
c. Skills in conflict resolution
d. Interagency and referral skills and experience.
