Medicaid Waiver Case Managers-RN and LSW (MS)

POSITION REQUIREMENTS: Must be a licensed RN or LSW with a Bachelors degree with at least 2 years nursing or social work experience. Previous Case Management experience preferred. Excellent communication skills and ability to work with elderly and disabled required.
POSITION SUMMARY: Assesses and plans care while maintaining complete, standardized documentation of client's progress and interaction with service providers, including all applications and forms required.
